Please support us with the following information:

1. Your account number or billing name and address
2. Your shipping address and any "attention to" instructions

4. The item Product Code from this Pricing Schedule, the description and number of cases you wish to order
5. "ShipVia" or delivery instructions
6. Any other special instructions
7. Sales tax exemption certificate if applicable.

Minimum Order Quantity
All products have a minimum order requirement of single case quantity.

Payment Terms
Payment terms are 1% 15 Net 30.

Finance Charge
A 1% finance charge may be assessed each month on any unpaid balance from the 31st day from the date of the invoice.

Freight Policy – Rush/Overnight Shipments
All shipping charges, including charges for expedited or overnight shipping will be prepaid and added to your invoice. 

Goods Damaged in Shipment
All goods should be inspected immediately upon receipt.  Retain all packing and shipping papers.  Goods damaged in shipment are the responsibility of the delivering carrier who should be contacted immediately if damage occurs.

Returned Goods Policy
Dated products or products shipped sterile are not returnable.
